A car radiator leaks because either the radiator hose is punctured or the radiator drain plug O-ring is damaged.

As per the problem mentioned it's definitely possible that the drain plug could be leaking. The plug is plastic and it should have two O rings on it. Design will vary a little bit if the van has had a replacement radiator at some point, but either way it should have at least one O ring, usually two. 

Any other possibilities other then faulty drain plug?

It's not unlikely that the plug is leaking but there are other things in the area that can leak too. The upper radiator hose is on that side, and although it's higher, it could leak down to the drain plug area. If the radiator side tank seal or the radiator itself was leaking it could also be wet in the same area.

Why Coolant is Seen Leaking?

If the plug was leaking the coolant would be coming out from around it or from the small drain tube directly under it. If that's where it is coming from and the other areas look ok then go ahead and drain the coolant and remove the plug.

You should turn off the engine of your car and wait until the engine cools down completely before you can work on its cooling system.

The plug is plastic and sometimes the threads no longer hold it tight enough to seal. You may find the threads on it are damaged when it is removed. 

If you take the plug to a parts store or dealer they'll be able to get you a replacement plug or replacement O rings. The most common thing to do would be to replace the entire plug, which is only a few dollars. I've tried to match up O rings in the past to just replace the rings and not the plugs, and the O rings are often slightly bigger or smaller than the ones you can buy in bulk.

1994 Toyota Camry: Coolant Leak?

Coolant leaking has multiple possibilities.
First inspect the radiator.The pipes/tubes from which coolant passes, Also confirm that there is no coolant getting mix with engine oil or transmission oil.
It can be worn out or faulty Radiator cap.Due to which pressure gets released.And coolant leaking from radiator cap.
Also make sure that there are no leaks getting to the ground anywhere? Is the carpeting getting wet with coolant in side the vehicle?
Properly check this basic possibilities.In most of cases this possibilities cause coolant leak.

If all options have been exhausted to find an external leak from any of the hoses or radiator when 13 up to 15 pounds of pressure were put on the system this only leaves two possible options.
The first one being under the intake manifold is a coolant pipe that is known to leak from the seals. Over time the coolant leaks out but is never enough to push out over and come down onto the ground it usually evaporates creating a crust build up under the intake.
The second possible option is that the head gaskets are starting to leak slowly and the coolant is being burned and sent out the exhaust. This is a small amount and generally you will never see any excessive steam from the tailpipe. Eventually it will start to leak to the point where it will start to overheat. Given the age and probable high mileage unfortunately you're probably looking at leaking head gaskets.
This is not an oil mixing with water type of head gasket leak typically this engine leaks between the combustion chamber and the coolant jacket.

Temperature gauge keeps fluctuating up and down close to red?

This problem is noticed on many car models.In this case the problem is noticed on 1997 Mazda 626.
First of all,turn the engine off and see if you notice any coolant leak.If yes, then that is the cause of problem.The coolant level is getting low and causing the temp gauge to fluctuate low to high in red zone.


But if there is no coolant leak,then other possibility can be faulty thermostat.
That could be the thermostat, you may want to take it out and check it in hot water to see if it opens, however,

With any engine that has been run hot, over heats or has been run hot, the first thing to is check the integrity of the internal engine, (ie. Head gasket(s) and head(s) and block) . Then consider the radiator being restricted somewhat.

You can check for cool spots in the radiator’s core that would indicate a clog.
The first thing that you want to do is check the cooling system for any exhaust gases. There is a tester for that, that you can get from Autozone service center for about $20. It comes with instructions, but there is a blue fluid that is used in it, if the fluid turns yellow, that is exhaust gases.

If there are any exhaust gases in the cooling system, that will cause more pressure than desired and will allow the engine to burn coolant and/or heat the coolant up too high. Most times not filling the crankcase with coolant either.

If there are, then there would be an internal engine problem, like a head gasket or head.

Of course, you will want to make sure that the system is full, the coolant is circulating, and the cooling fan(s) are working as well as the pressure cap holding pressure.
